Licensed Practical Nurses (LPNs) are valued members of our health care teams, providing a wide range of care and service. With a focus on promoting wellness and preventing illness, LPNs work to support the diverse needs of our patients and families. LPNs are one of the collaborators in the Collaborative Care Model. As a member of the patient's healthcare team, the LPN applies practical nursing theory in the assessment of patients, collaboration in the development of the nursing care plan, implementation of the care plan and ongoing evaluation of the patient. The LPN provides nursing services independently for patients  under direction or guidance of a registered nurse, medical practitioner, or other health care professional.
